Admission Process
In most cases physicians or other health care professionals refer Hospice of South Central Indiana patients. However, anyone can refer patients including family and friends. The Hospice Intake Coordinator will ask general information including permission to contact the patient’s personal physician to make sure the patient is eligible for Hospice services. A time will be arranged for a Hospice staff member to visit with the patient and/or family to explain how Hospice can help. If Hospice is the service you want, Hospice will take care of the rest.
